Directs and coordinates continuum of care operations to ensure the
effective transition of health care services for patients.
Responsible for directing and leading a regionwide approach in the
placement of patients with complex medical needs from acute care to
appropriate levels of care settings.
- Directs the continuum of care operations across the medical
center ensuring well coordinated plans of medical treatment and the
cost effective and efficient utilization of healthcare services and
resources. Oversees and coordinates the utilization review and
management of patient treatments, resources and social
- Develops and implements the full integration and
standardization of continuum of care services to improve the
transition of patients along the continuum of care. Evaluates and
improves the quality and appropriateness of care. Collaborates with
leaders to develop and implement innovative continuum of care
models and best practices. Promotes new ways of enabling patient
access to all levels of care.
- Plans for anticipates and future issues or problems for
patients that have been identified with complex needs and have
lengthy of stays in the acute care setting. Provides regionwide
approach and direction in handling patients with complex needs.
Leads a multi-disciplinary team of physicians, nurses, leaders,
legal counsel, and social services professionals to ensure the
appropriate placement and transporting of patients with complex
needs from the acute care setting to other levels of care. Develops
improvements and strategies managing risk and cost associated with
placement of complex patient needs.
- Works with leaders across the continuum to recommend
contracting relationships for outside care services when necessary.
Monitors, tracks and analyzes trends in patient
- Monitors medical center performance of goals and standards of
patient continuing care. Monitors compliance with regulatory
agencies. Develops objectives and policies. Manages
- Minimum ten (10) years of experience in a clinical or
management function in a multi-faceted healthcare system and
multiple healthcare service provider setting.
- Bachelor's degree in health services, business administration,
public health, nursing or related discipline required.
License, Certification, Registration
- Current California license required in any of the following
processions: Registered Nurse, Pharmacist, Clinical Social Worker,
Phsyical Therapist, Occupational Therapist, Speech Therapist or
- Demonstrated success in meeting or exceeding performance
- Expertise in project management.
- Proven leadership skills with ability to influence and manage
- Demonstrated ability to function collaboratively in a
multi-entity partnership environment.
- Requires ability to manage various stakeholder interests and
demonstrated skills in facilitation, problem solving, decision
making, persuasion and team-orientation.
- Thorough knowledge of quality assurance, resource management,
health plan regulatory and licensing processes including federal
and state laws and regulations such as: Knox-Keene Act, Federal HMO
Act, Public Employees Medical and Hospital Act, The Joint
Commission, and all applicable Medicare and Medi-Cal
- Effective written and oral communication skills with ability
interface with all levels of management.
- Must be able to work in a Labor/Management Partnership
- Master's degree in health care, public administration or
