What You Will Do to Change Lives
Under direct supervision, serves as a primary support and case
manager for individuals with mental illness by assisting them to
live as independently as possible in their chosen community. In
addition, this position provides knowledge of a specialty area to
the treatment team and program.
Shifts Available:
On Call; AM Shift Hours and Days vary as needed
Wage range $20.94 - $30.23 - Telecare applies geographic
differentials to its pay ranges. The pay range assigned to this
role will be based on the geographic location from which the role
is performed. Starting pay is commensurate with relevant experience
above the minimum requirements.

In partnership with DSH and CONREP, the Telecare Alameda FACT
program serves 60 justice-involved adults with mental health
challenges and support members to step down to, and successfully
remain in, the community after an extended stay in the State
Hospital or other long term mental health facility. Our program
uses a community-based model where we connect members to the proper
resources, including safe housing, to help them achieve their
wellness goals. We will also offer psycho-educational, peer/family
support, and skills building groups as well as medication
management services.
