Job Description:

A company is looking for an RN Case Manager - Remote.

Key Responsibilities:

Meet with patients, families, and caregivers to discuss care and treatment plans
Consult with the care team to ensure successful implementation of care plans
Monitor and coach patients using motivational techniques to maximize self-management

Required Qualifications:

Graduation from an accredited school of nursing
Active, unrestricted Registered Nurse license through the State of California
1+ years of experience in a clinical setting or care management
Ability to work Monday - Friday, 8-5 PST
Bachelor of Science in Nursing (BSN) preferred
