DEPARTMENT OVERVIEW

University of California (UC) Health provides leadership and strategic direction for UC s five medical Centers. UC Health supports operational initiatives at individual campuses and development of systemwide initiatives, promoting collaboration among the university s health professional schools and providing oversight on the business and financial activities of the clinical enterprise.

POSITION SUMMARY

The Population Health Specialist manages a portfolio of projects that promote UC Health as a leader in healthcare innovation and achieving the triple aim. Facilitation of systemwide opportunities are in support of the UC Health Quality / Population Health Program and its strategic plan. By exercising advanced organizational and project management skills, you will engage with faculty, clinicians, staff and stakeholders inside and outside of UC Health. As a lead facilitator, you will help develop and implement high-impact, system-wide programs that guide and support a broader strategic direction for UC Health. An ideal candidate for this role has demonstrated success in leading complex collaborations and applies knowledge of population health management practices to substantial projects of large scope and with a high degree of complexity.

Responsibilities

Analyzes and interprets highly complex data of broad scope, and summarizes findings. Develops corresponding reports, presentations, and action plans. Prepares and delivers executive summaries.

Leads systemwide population health, delivery system innovation, and alternative payment model projects with significant complexity and organizational scope.

10%

Guides the implementation and standardization of selected systemwide interventions. Develops and distributes work tools and best practices to support performance improvement.

Partners with the UC Health Center for Data Driven Insights & Innovation (CDI2) and Self-funded Health Plans teams to develop benchmarks to measure short and long-term impact of innovations. Compares performance against benchmarks; provides proposals and plans to senior leaders to improve program.

Researches best practices and develops and implements improvements in the data collection and reporting processes and systems.

Tracks and reports various clinical performance improvement data and statistics to leadership. Administers surveys, conducts direct observations, and interviews stakeholders.

Working with the Program Director and Quality and Population Health team, develops innovative implementation plans with stakeholders, tests assumptions and develops contingency plans.

Required Qualifications

* In-depth knowledge of population health, delivery system reform, and accountable care standards.

* In-depth knowledge of health program / policy analysis. Advanced ability to produce detailed analytical reports.

* Experience implementing and / or evaluating population-based health programs.

* Advanced organizational and project management skills, with the ability to lead collaborative workgroups, prioritize tasks, and direct projects from inception to completion.

* Advanced critical thinking and problem-solving to evaluate highly- complex information, assess problems, and develop effective solutions.

* Advanced interpersonal skills for collaborating effectively with clinical and professional staff.

* Advanced written and verbal communication to convey complex information and produce high-quality reports and presentations.

* Strong knowledge of Microsoft Office Suite, and specialized applications such as Adobe Acrobat and Visio.

* Advanced ability to influence and persuade staff, management, and clinicians across departments.

* Advanced ability to exercise professional judgment with sensitive and / or confidential issues.

Preferred Qualifications

* Master's degree in health-related field or relevant profession preferred.

Education

* Bachelor's degree in health-related field or relevant profession, or a combination of comparable clinical education and / or experience.
