Overview:The Medical Coordinator is responsible for tracking the medical needs and services of the youth in care across all RFA programs. The Medical Coordinator works with a Nurse or Nursing Supervisor to ensure all needs are properly assessed and addressed in a timely way and performs program support duties focused on ensuring all youth in placement have up to date medical visits and records. The Medical Coordination duties are focused on ensuring all youth have doctor and dental appointments at the appropriate time and charts are up to date. The Medical Coordination also ensures all caregivers are completing and submitting necessary medical records, including medication administration records and tracking psychotropic medication paperwork as needed, including court authorizations.Seneca Family of Agencies is committed to providing traditionally marginalized communities a network of outstanding mental health, community based, and educational services. Our programs are deeply engaged in conversations and trainings on Diversity, Equity, and Inclusion in order to bring equity and justice to the youth and families we serve.Responsibilities:Obtaining pertinent medical information on new clients and throughout placement and alerting nursing team to any issuesEnsuring all medical data is collected and youth charts are up to dateSupport medical appointments being scheduled in accordance with regulations and in collaboration with direct care and familiesAdministrative work (copying, filing, telephone, computer processing) in relationship to medical care, especially as related to obtaining records from medical appointments and court authorizationsCollaborate as part of a multidisciplinary teamQualifications:Bachelor's degree required.Valid California driver's license with a clean driving record.
