This role actively engages with patients both in the home and by telephone. The incumbent will use compassion and empathy to earn patients' and caregivers' trust, persistence to support and advocate for them, and excellent communication skills to build impactful relationships. -This position is full-time, Monday - Friday. Employees are required to work our normal business hours of 8:00am - 5:00pm. It may be necessary, given the business need, to work occasional overtime or weekends.Primary Responsibilities:Conduct home visits and/or phone or video calls to patients to collect clinical and psychosocial information, and carry out designated tasks, including but not limited to:

  • Resource referral and connection
  • Quality measure documentation, support, and follow up
  • Telemedicine facilitation and set up
  • Collect information regarding patient conditions using standardized checklists and templates.
  • Partner with Landmark interdisciplinary team (providers, nurses, social workers, etc.) to drive improvement in patient outcomes and satisfaction
  • Actively participate in weekly interdisciplinary team meetings You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:
    • High School Diploma (or higher)
    • 3-5+ years of experience in any of the following job categories or categories similar to: Social Worker; Home Health Aide; Certified Nursing Assistant; Certified Medical Assistant; Medication Aide; Health Coach; Health Navigator; Healthcare Technician -
    • Ability to conduct independent home visits
    • Experience with Microsoft Office products (Word, Outlook, and Excel)
    • Reside within South San Francisco or Oakland area
    • Required to have a dedicated work area established that is separated from other living areas and provides information privacy
    • Ability to keep all company sensitive documents secure (if applicable)
    • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
    • Access to reliable transportation required; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety policy. -
    • Ability to work our normal business hours of 8:00am - 5:00pm.
