TITLE: -Manager Clinical Services (MCS) - Hospice
OFFICE LOCATION: -Oakland, CA -
SCHEDULE: -Full Time
SHIFT: -M-F
The posted compensation range of $130,302 - $170,234 (annual
salary) is a reasonable estimate that extends from the lowest to
the highest pay Pathways Home Health & Hospice in good faith
believes it might pay for this particular job, based on the
circumstances at the time of posting. Pathways Home Health &
Hospice may ultimately pay more or less than the posted range as
permitted by law.
POSITION SUMMARY: -The Manager, Clinical Services (MCS) contributes
to the organizational success of providing high quality care by
ensuring the coordination, continuity and quality of clinical care
following a physician's approved plan of treatment. The Manager,
Clinical Services facilitates the completion of documentation to
comply with regulatory, operational and financial requirements. The
Manager, Clinical Services acts as a clinical resource to internal
and external customers. The Manager, Clinical Services works
independently with limited direct supervision, follows and manages
staff to adhere to company guidelines, policies and procedures and
acts in good judgment.
AREAS OF RESPONSIBILITY:
1. Adheres to the Corporate Compliance Program, including
confidentiality of HIPAA protected health information. Maintains
confidentially of patient information.2. Responsible for processing
assigned workflow in a timely accurate manner. Demonstrates the
ability to be efficient and productive by organizing job duties and
responsibilities and is readily and professionally available to
assist all office and clinical staff.
3. Enters and approves all orders for tracking purposes and assure
sent to physician for signature. Ensures current orders exist for
requested medical supplies. Assures missed visits reported to
physician.
4. Reviews and processes vital sign alert reports, documents
action, including any follow-up and physician notification. Review
and process all wound score deviations documenting any action
and/or follow-up.
5. Supports and promotes department, branch and organization
decisions. Functions as an effective team member and contributes to
the development and achievement of branch and organizational goals
and objectives.
6. Creates an "expectation of excellence" and manages performance
of direct reports to maximize the value of associates'
contributions towards organizational success.
7. Establishes strategically aligned goals with associates and
creates opportunities for continuous improvement of staff by
providing frequent coaching, mentoring, and accurate performance
feedback. By providing direction and support to staff to deliver
quality services with a focus on organizational success. Provides
fair and equitable treatment of all associates.
8. Ensures appropriate staffing levels are maintained, by
participates interviews, orients direct reports, recruiting and
retaining qualified, productive and customer-focused associates as
well as monitors recruitment/retention activities and costs.
Provides education and training for associates in support of career
goals and the organization's mission and strategies.
9. Maintains positive staff morale, observable through team spirit,
a sense of belonging and recognition of accomplishments and creates
a high performing work environment characterized by transparent and
open communication, mutual trust and respect.
10. Maintains effective communication.
11. May be required to perform patient visits.
12. All other duties as assigned.
QUALIFICATIONS:
1. Current Registered Nurse (RN) license (BSN preferred) in good
standing to practice in the state(s) is required or Doctorate of
Physical Therapy
2. -Two years Hospice nursing experience is required
3. Excellent clinical skills.
4. Thorough knowledge and understanding of home care regulatory
requirements.
5. Compliant with appropriate state certification/registration.
6. Strong written and verbal skills.
7. Ability to work closely with a wide range of professionals or a
multi-disciplinary team. Interpersonal skills that facilitate
inter- and intra-team/departmental communication.
8. Demonstrated leadership, administrative and supervisory
skills.
9. Excellent organizational and time management skills.
10. Excellent attention to detail is required.
