• Conduct assessments and evaluations to determine patient needs and progress
• Collaborate with other healthcare professionals to ensure comprehensive patient care
• Document patient progress and maintain accurate recordsRequirements
  • 1 year of recent experience as a Physical Therapist in a SNF / LTC setting
