: Registered Nurse - Case Manager Location : Santa Rosa, CA 95405
Shift : Day 5x8-Hour (08:00 - 16:30) Qualifications : Minimum of 2
years of acute hospital case management experience in care
coordination and discharge planning. Certifications: Basic Life
Support (BLS) certification required; additional certifications
preferred. Skills : Experience in acute hospital settings,
ambulatory care, behavioral health, home health, hospice, and
long-term acute care. Knowledge of CPT coding, ICD-10 coding, and
DRG. Competency in computerized documentation and data abstraction.
