Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks
Start Date: ASAP About the Position Title:, Marin County,
California Role: Physical Therapist - Home Health Role
Responsibilities: Required to prioritize start of care visits
Required to supervise Physical Therapy Assistants Next day
mandatory reporting of start of care visits Clinical notes and
other documentation due within 48 hours of visit Maintain timely
communication with supervisor, intake, and office Be generally
responsive during normal business hours Requirement: CA PT License
and one year of Home Health experience Territory: Marin, Sonoma,
and Napa County, Ca (Will be required to expand as needed)
Schedule: 40 hours per week, Monday-Friday and one weekend a month
rotation Training: 3 full days, one in house and two in the field
Documentation System: Kinnser Equipment Issued: iPad, Verizon One
Talk Mileage Reimbursement: $0.65.5/mile (from first to last
patient) Parking and Toll Reimbursement: Paid Company Car: Option
with a decreased bill rate Bill Rate: $105 OT Rate: $5, paid after
30 points per week Holiday Rate: $10 Weekend Rate: N/A On-Call
Rate: N/A Call Back Rate: N/A Productivity Requirement: 30 points
per week Point to Hourly Conversion: 1 = 1.33hrs *Caseloads and
visit mixes per day will equal to an average of 8 hrs per day and
40 hrs per week. Overtime must be pre-approved by the Clinical
