Child and Adolescent Care Navigator

The current vacant position is grant funded for a limited duration and shall not exceed twelve (12) months or when the funds are exhausted, whichever comes first.

PRIMARY PURPOSE: The Child and Adolescent Care Navigator is responsible for linking patients who have experienced adverse childhood experiences (ACEs) to appropriate follow-up resources, including medical and mental health services as well as social determinant of health resources. The Child and Adolescent Care Navigator will use the FINDconnect program and other resources. This position serves as the liaison between the patient and the community and is the point person for child and adolescent care navigation for Davis and Transit Village pediatrics sites.

ESSENTIAL FUNCTIONS:

* Advocate for patients referred by PCP and/or other provider and help them achieve their best health. * Create a care plan that reflects patients and familys needs. * Connect patients to resources to meet these needs, primarily utilizing the FINDconnect Program * Serve as a liaison between the community and health and social services. * Be highly dependable and independent much of the work will take place in various settings with the possibility for meeting patients in a community setting or in their home. * Communicate well with patients, document clearly in the medical record and interface with medical providers to advocate for the patient. * Commit to ongoing trainings on ACEs. as well as to collaborate with other key players in this space * Support a positive work environment and foster teamwork.

KNOWLEDGE, SKILLS AND ABILITIES

Competencies

* Competent leadership and administrative skills. * Flexibility and creativity this is a pilot program and will be an iterative and evolving process. * Good communication and customer relations skills; ability to work well with a team and independently. * Proficiency in computer operations, navigational skills and comfortable with Internet-based applications, Microsoft Word, and Excel. * Sound decision-making skills including problem solving, critical thinking, and good clinical judgment for clinical and non-clinical issues. * Professional demeanor. * Cultural humility and respectful curiosity, ability to meet patients and families where they are.

Essential Requirements

* Excellent communication, organizational and time management skills with the ability to meet tight timeframes. * Knowledge of health issues and the health care system * Ability to work effectively and collaboratively with a variety of customers including hospital/office staff, health plans, members and physicians. * Flexible and adaptable to change. * Ability to learn quickly and retain complex information. * Reliable transportation * Constructive in interpersonal relationships * Shows leadership in the community * Able to identify and use resources (e.g. having initiative, being self-directed)

MINIMUM EDUCATION

* High school diploma/AA or BA preferred. * Minimum Experience: Prefer (though not mandatory) 1-2 years of experience as a navigator/ coordinator in community based setting. * Language: Fluency in Spanish (both written and spoken).
