Job Description:

Under the supervision of the Contra Costa Regional Manager, the full time Program Specialist will provide oversight and coordination for two new grant-funded programs at La Clinica de La Raza's sites in Contra Costa County: Activating Care for Contra Costa Seniors (ACCS) and Veteran Health Navigation Partnership (VHNP).

The ACCS Program will enhance care coordination for low-income seniors in Contra Costa County including implementation of a robust screening program for high-priority health issues of mobility, cognition, and depression. The overall goal of the ACCS Program is to help senior patients improve their mental and physical health through timely identification of problems and enhanced linkage to appropriate and well-coordinated care.

Core duties for ACCS include: (1) training and supervising two full time Senior Care Case Assistants; (2) coordinating and monitoring all enrollment, screening, assessment, referral and access to care activities including documentation in the electronic health record and the population health management software; (3) building capacity for improved senior care services; and (4) working collaboratively with integrated care teams. The Program Specialist ensures coordination of staff training, collaborative meetings, and monitors and tracks staff progress achieving grant deliverables and outcomes.

The Veteran Health Navigation Partnership (VHNP) will enhance care coordination for veterans, particular female veterans and other underserved veterans, in East Contra Costa County. The overall goal of the VHNP is to increase access to quality health care for all veterans through coordinated outreach, education, connections to community resources, and provision of integrated health care services at La Clinica.

Core duties for VHNP include (1) coordinating process for La Clinica to become a registered Veterans Choice Program (VCP) provider; (2) developing policies, procedures, and workflows for communication and exchange of documents with the VA; (3) building capacity for improved veteran services; (4) working collaboratively with Peer Navigator and Case Manager; and (5) convening and facilitating meeting with VHNP partners.

Both programs encompass the following three primary care sites operated by La Clinica de La Raza: Monument Medical, Pittsburg Medical and Oakley Medical. The Program Specialist will be expected to work travel between all sites.


* Coordinate and lead La Clinica's ACCS program-Activating Care for Contra Costa Seniors * Coordinate and lead La Clinica's participation in the Veterans Health Navigation Partnership (VHNP). * Develop an implementation plan, timeline and workflow for project activities related to ACCS and VHNP. * Develop, implement and evaluate the effectiveness of the ACCS and VHNP programs. * Meet regularly with the ACCS staff for orientation, training, coaching, feedback and performance monitoring; work closely withstaff to successfully recruit seniors for the program and link patients to appropriate services. * Meet regularly with the Peer Navigator supporting VHNP to coordinate program activities and successfully recruit veterans for the program. * Enhance capacity for access to senior and veterans services by building upon and developing new community partnerships and conducting outreach to senior-focused and/or veteran-focused organizations and service providers. * Research available community resources for both programs and educate La Clinica's providers about available resources. * Work with providers and leadership to explore opportunities for further growth and improvement of La Clinica's senior and veterans services to build capacity for continuing growth in the number of senior and veteran patients served. * Monitor data gathering and documentation processes, services delivered and coordinate program evaluation. * Manage any contracts made with outside organizations * Track expenses made for patient supplies * Participate in relevant meetings * Other duties as assigned.

Required Skills

* Bilingual English/Spanish preferred but not required * Master's Degree in Social Work, Counseling, Psychology, Education, Public Health, or Bachelor's Degree with (1-2) years related experience * Strong organizational skills. * Keen attention to detail. * Excellent interpersonal skills, verbal and written communication skills. * Self-starter with the ability to work independently. ? Excellent computer skills. * Ability to work as part of a team and inspire collaboration and partnership. * Must have a motor vehicle available for daily use, possess a valid California driver license acceptable to La Clinica's insurance company, and carry vehicle liability insurance with at least $100,000/$300,000 limits.

Required Experience

* 1-2 years of the following * Interest in or experience working with seniors and/or veterans in a community setting. * Experience with supervising staff. * Must be committed to addressing issues of access to care barriers and social stigma related to seniors and veterans. * Knowledge of and respect for underserved communities in Contra Costa County. * Experience with community outreach and engagement methods. * Experience in program planning, implementation, and evaluation
