Job Summary

The UCSF Benioff Children Hospital's Child & Adolescent Psychiatry Portal (CAPP) is aimed at expanding pediatric primary care providers' (PCPs) knowledge, skills and comfort with mental health screening, diagnosis, and treatment, in order to better address and improve access to behavioral health treatment in our communities. CAPP child & adolescent psychiatrists provide real-time, direct connect consultation and case-specific guidance to pediatric PCPs on screening, assessment, diagnosis, treatment planning, and access to resources and referrals. CAPP also offers continuing medical education programming events for PCPs, in order to improve their knowledge and comfort in managing pediatric mental health conditions, as well as resources on our website.

In collaboration with the interprofessional team, the Referral Coordinator Specialist will serve as a clinical liaison who helps to further assess and triage family needs, including providing parent guidance and support as a bridge to longer term care. The Referral Coordinator Specialist will be responsible for further identifying clinically relevant and accessible resources, and coordinating patient/family linkage to care with the identified resources and referrals which are part of treatment recommendations.

In consultation with the clinical leadership team, the Referral Coordinator Specialist will also be responsible for identifying, organizing and managing local and regional clinical referral resources, and ensuring these are appropriately accessible to all necessary stakeholders, including patients and families, enrolled members i.e. primary care and other providers, as well as care team members organized along a continuum of care, with information about severity, types of treatment offered, and insurances accepted. The ideal candidate will have demonstrated leadership and initiative in clinical resource development, and in using data to guide decision-making, as well as demonstrated project management abilities.

The Referral Coordinator Specialist will support staff through website development, with special emphasis on cultivating and organizing local and regional clinically relevant resources and creating and maintaining an essential resource database, as well as community outreach.

Required Qualifications

  • Master's degree from an accredited school of social work, possession of a clinical social worker license issued by the California Board of Behavioral Science Examiners, and three years of post-master's experience; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position
  • Meets requirements necessary for credentialing at the UCSF Medical Center
  • Demonstrated clinical training and experience (at least 1 year) in pediatric mental health conditions, including helping children and families (1) manage anxiety, depression, ADHD, post-traumatic stress, developmental concerns, including both typical development, and also disabilities and delays, as well as other common mental and behavioral health conditions and (2) develop and strengthen coping skills
  • Knowledgeable about pediatric mental & behavioral health landscape in the local and regional Bay Area and beyond, with experience in working with community agencies
  • Knowledge of computer systems and software used in functional area
  • Must be comfortable using data to drive decisions around program initiatives, including monitoring, analyzing, forecasting, and reporting
  • Strong relevant subject matter knowledge
  • Demonstrated experience working independently
  • Demonstrated ability to work productively and communicate effectively as a member of a multi-disciplinary team
  • Self-directed and effective in problem solving, systems planning and patient care management in a high-volume work environment
  • Excellent interpersonal, written and oral communication skills
  • Ability to direct the work of others, for jobs requiring supervision
  • Demonstrated ability to establish priorities, manage shifting priorities, and handle multiple time-sensitive clinical and operational issues
  • Ability to accomplish goals working through formal and informal channels, with diplomacy and tact
  • Demonstrated creativity, problem solving, critical analysis, initiative, judgment and decision-making skills

Note: Physical/health screening required.

Preferred Qualifications

  • Experience in a high-profile staff role, where influence and communication skills are critical to accomplishing goals
  • Experience with culturally, ethnically and socioeconomically diverse populations
  • Knowledge of, and experience with, trauma-informed care and/or trauma-informed systems
  • Experience in pediatric integrated health care settings
  • Experience with Epic or other EHR systems
