Job Description: The Pediatric Patient Navigator serves as a
non-clinical resource, advocate, and liaison for patients and their
families and is an integrated member of the team. The Patient
Navigator collaborates with colleagues and managers to meet patient
expectations, achieve quality outcomes, and build a
patient-centered culture.# The Patient Navigator works in a
non-judgmental and respectful manner, closely with families whose
children are receiving medical and psychosocial services within the
department, while assisting the family in accessing needed
services, promoting independence and advocacy skills within the
family.# # Position Requirements: Minimum Education:# High School,
GED or equivelent required. Associate#s or Bachelor#s Degree
preferred. Minimum Experience:# Two (2) or more years of experience
as a patient navigator, Community Health Worker, or relevant
experience in patient-oriented support, preferably in an outpatient
medical clinic or physician#s office. Certification Required:#
Patient Navigator or Community Health Worker (CHW) Certificate, or
completion of the certificate within three months of employment, is
preferred. A combination of relevant education and training may be
considered in place of Certification. Knowledge, Skills, and
Abilities:# Demonstrated experience prioritizing tasks and
following-through with minimal supervision.# Adept at organizing
and tracking information.# Able to work in a busy team
environment.# Familiarity with insurance authorization and referral
processes preferred.# Experience working with children and adults
with medical, behavioral health, or developmental challenges who
has previously or is presently utilizing services.# Experience
navigating county child serving systems. Good skills at working
within a team.# Shows ability and interest in addressing the impact
of culture/ethnicity and language on the provision of accountable
and sensitive services to diverse populations.# Ability to work
occasionally on evenings or weekends (on average once a quarter or
four (4) times per year).# Experience working with people from
diverse backgrounds. Ability to work effectively with large
bureaucracies.The position requires a degree of independence,
initiative, social skills, and emotional intelligence in order to
support patients with a wide variety of needs, effectively
communicate with clinical staff, and provide concise written
documentation of patient encounters. Technical Knowledge:
Proficiency with Windows-based software including Excel, Microsoft
Word, and databases, locating information on the Internet, and
ability and willingness to learn new software. Prior experience
with Electronic Medical records (EPIC) preferred. Experience with
extracting data from patient medical records and other types of
databases preferred. Knowledge of medical terminology.# # Physical
Requirements: Sit: Up to 8 hours/day Stand/Walk: 6-8 hours
Bend/Stoop: Up to 3 hours Reach: Up to 6 hours Rep Use of UE/Grasp:
